Practical takeaway

The default for most people:
• Get regular, non-burning sun — short, frequent exposures (especially morning light for circadian/mood; some midday skin exposure for vitamin D in season/latitude). Build up gradually; never to the point of redness.
• When you do need sunscreen (prolonged outdoor time, high UV index, fair/high-risk skin, midday summer), prefer mineral (zinc oxide / titanium dioxide) — reapply diligently (mineral can rub off; modern micronised ZnO is cosmetically much improved).
• Don't reflexively apply chemical SPF indoors/in winter — that's the over-generalised part. (Caveat: UVA does penetrate glass and drives photoaging, so high-exposure window-seat/driving situations are a reasonable exception.)

Reserve heavy/daily high-SPF for genuine high-UV-index, prolonged outdoor exposure, outdoor occupations, and high-risk skin (very fair, lots of moles, personal/family melanoma history) — there, daily protection clearly earns its place.

Why This Entry Exists

Mainstream advice has collapsed to a slogan — "wear broad-spectrum sunscreen every day, indoors and out" — and it's been repeated so often it reads as settled. It isn't wrong, but it's oversold, and the over-generalisation hides three things a thoughtful person should weigh: chemical UV filters reach the bloodstream at meaningful levels, sunlight does things (circadian entrainment, nitric-oxide-mediated blood-pressure lowering, mood) that a vitamin-D capsule does not reproduce, and categorical sun avoidance is itself associated with higher mortality. Meanwhile the opposite camp — "sunscreen is toxic, the sun is medicine" — overshoots in the other direction and ignores that the one sunscreen RCT genuinely reduced skin cancer.

This entry holds the honest middle. It's deliberately cross-pillar because sun touches skin, vitamin D, circadian rhythm, and mood at once.

What bad advice this protects against, both ways:
• "Slather chemical SPF daily, avoid all sun" → unnecessary year-round chemical-filter exposure, suppressed sensible sun, and the downsides of true avoidance.
• "Sunscreen is poison; sunbathe freely; take a vitamin D pill instead" → burning (the actual melanoma driver), and a pill that doesn't replicate sunlight's non-vitamin-D effects.
• Treating "the sun" as one thing → conflating sensible non-burning exposure (beneficial) with burning/high-UV overexposure (harmful).

Evidence

1. Only mineral filters are FDA-GRASE (Tier 1, regulatory). In its 2019 proposed rule, the FDA classified only zinc oxide and titanium dioxide as Generally Recognised As Safe and Effective (up to 25%). The 12 organic ("chemical") filters — oxybenzone, octinoxate, avobenzone, homosalate, octocrylene, octisalate — were not deemed GRASE pending more data. "Prefer mineral" is the conservative, regulator-aligned default, not a fringe view.

2. Chemical filters are systemically absorbed (Tier 1). FDA maximal-use trials (Matta et al., JAMA 2019 & 2020) found all tested organic filters exceeded the 0.5 ng/mL toxicology-threshold-of-concern after a single day; oxybenzone reached ~180–258 ng/mL plasma and persisted for days. Mineral filters (ZnO/TiO₂) do not meaningfully penetrate the stratum corneum. The absorption contrast is real.

3. …but human harm is unproven — precaution, not demonstrated injury (Tier 3). Systematic reviews show estrogenic/antiandrogenic/thyroid signals for oxybenzone in vitro and in animals, but human epidemiology (with ~97% population biomonitoring detection) has not corroborated adverse health outcomes at typical exposure. So "prefer mineral" rests on precaution given proven absorption, not on demonstrated harm.

4. Sunscreen genuinely prevents skin cancer — the core consensus is sound (Tier 1). The only sunscreen RCT — the Nambour trial (Green et al., n=1,621) — found daily sunscreen cut squamous cell carcinoma ~40–60% and, at 10-year follow-up, halved invasive melanoma (HR 0.50; borderline, P=.051). So "never use sunscreen" is not the takeaway; "never burn, and protect during real UV exposure" is.

5. A vitamin-D pill does not replace sunlight (Tier 1–2). VITAL (Manson et al., NEJM 2019, n=25,871) found 2000 IU/day vitamin D did not reduce cancer incidence or major cardiovascular events — undercutting "just take a pill instead of sun." Sunlight has vitamin-D-independent effects: UVA mobilises cutaneous nitric oxide, lowering blood pressure (Weller/Edinburgh), and morning light entrains the circadian clock and lifts mood (see circadian_rhythm_optimization, morning_sunlight_exposure).

6. Sun avoidance carries a real mortality cost (Tier 2, observational). The Lindqvist/MISS cohort (n=29,518 Swedish women, 20-year) found sun avoiders had ~2× all-cause mortality vs the highest-exposure group, an effect the authors likened in magnitude to smoking. It's observational (reverse-causation possible) — so it rebuts categorical avoidance without licensing deliberate high-dose sun-seeking.

7. Daily sunscreen rarely tanks vitamin D in practice (Tier 2). Real-world moderate-SPF use shows little vitamin-D reduction (people under-apply; incidental exposure persists); deficiency emerges only with SPF50+ near-perfect daily application (Sun-D RCT 2025). So the vitamin-D-suppression worry is a high-SPF/high-adherence edge case, not a population rule.

Mechanism

Burning vs cumulative dose. Intermittent sunburn (especially in childhood) is the dominant melanoma driver; steady non-burning exposure is a different risk profile and carries benefits. So "never burn" is the load-bearing rule — not "never see the sun."

Why mineral differs from chemical. Mineral filters sit on the skin surface and reflect/scatter UV (they don't absorb systemically); organic filters absorb UV and penetrate skin into circulation. Same SPF on the label, different systemic footprint.

Sunlight's non-vitamin-D effects. UVA liberates nitric oxide stored in skin → vasodilation → modest blood-pressure drop, independent of vitamin D. Morning light hits retinal cells that entrain the circadian clock (affecting sleep, mood, metabolism). A capsule supplies the vitamin-D molecule only — which is why VITAL's null doesn't refute "sun is beneficial," it refutes "a pill equals sun."

Behavioural "dependence." There's no physiological sunscreen addiction, but a daily-SPF-everywhere habit that displaces all sun exposure is a real behavioural pattern with a downstream cost (the Sun-D vitamin-D signal). That's psychological/behavioural dependence — real, not a myth — not a physiological one.

Risks And Contraindications

• Burning is the real hazard — protect against it (shade, timing, clothing, mineral SPF) without fearing all sun.
• High-risk skin (very fair, many naevi, immunosuppressed, melanoma history) should weight protection higher — the "sensible sun" default is for average-risk people.
• Chemical-filter precaution is about proven absorption with unproven harm, strongest for infants/children (higher surface-area-to-weight) and pregnancy → prefer mineral there.
• Over-avoidance has its own costs (mortality association, low vitamin D, circadian/mood) — don't swing to total avoidance.
• This is general guidance, not dermatological advice for someone with a skin-cancer history — that warrants a clinician.
